New Blood Test May Predict Alzheimer's Risk Years Before Symptoms Appear

The Blood Test Looks for an Early Alzheimer's Biomarker
The blood test that may change the face of Alzheimer’s research measures phosphorylated tau 217, or p-tau217, a form of the tau protein associated with Alzheimer's disease.
Researchers have known for years that abnormal tau proteins and amyloid plaques are two of the disease's hallmark features. According to existing studies, the p-tau217 biomarker starts reflecting changes in the brain long before cognitive decline begins. However, there hasn’t been a blood test that could accurately measure those changes until recently.
There are other reasons to be optimistic about this new test. Unlike PET scans or spinal taps, a blood test is relatively simple, less invasive, and potentially far more accessible if it eventually becomes part of routine clinical care.
The Study Followed Healthy Older Adults
The study, which was conducted by Mass General Brigham Neuroscience Institute, is the largest of its kind. It followed 2,700 older adults who were in “good health.” The pool of test subjects came from around the globe, including participants from North America, Japan, and Australia. Researchers followed the participants for years after collecting initial blood samples.
People with the highest p-tau217 levels faced an estimated 38% chance of developing cognitive impairment within five years. Over a 10-year period, that estimated risk climbed to 78%, suggesting the biomarker could provide meaningful insight into who is most likely to develop Alzheimer's symptoms in the future.
While the blood tests were at the heart of the study, the team also looked for ways to incorporate additional, existing tests with them. When additional screenings, including PET scans and lumbar punctures, were conducted, diagnostic accuracy jumped from 62% to 88%. These findings were officially presented at an Alzheimer's International Conference on July 14.
Researchers and Experts Say the Test Needs More Proof
The results of the test are certainly encouraging. However, the research team and experts in the field of cognitive health warn that it’s not a one-size-fits-all test. It’s worth noting that even members of the research team recognize that the test is not intended to tell healthy individuals with certainty whether they will develop Alzheimer's disease.
Instead, they believe it could become an important way to identify people who are most likely to benefit from future prevention strategies or participate in clinical trials testing therapies designed to delay the disease before symptoms appear.
Rachel Buckley, who served as the lead author in the study, said, "Our findings provide some of the clearest evidence yet that elevated p-tau217 levels may help detect dementia risk years earlier. Once verified, these blood tests could be used to recruit patients for clinical trials of treatments to prevent cognitive decline and dementia."
Despite the optimism and the acknowledgement from Buckley, some cognitive health experts have raised concerns. Dr. Reisa Sperling, who served as the senior author of the study, has been quoted as saying, “At this point it wouldn’t change what I would tell someone to do. I’d still tell them to eat well, sleep well, exercise a lot and stay engaged.”
Researchers note that factors such as age, genetics, kidney function, and overall health can influence biomarker levels, meaning additional studies are still needed before the test can accurately predict an individual's future.
The Future of Alzheimer's Research Is Shifting Earlier
